How to verify the E.S.PKU Metabolic Passport?
The information in the E.S.PKU Metabolic Passport is confirmed by the signature and stamp of the PKU doctor. The first page contains the name and address of the clinic and the name and surname of the PKU doctor. The passport provided officially will show a reflecting E.S.PKU logo if turned.
The QR code is linked to a landing page providing brief information on Phenylketonuria, MSUD, Tyrosinemia, Homocystinuria, Glutaric Aciduria Type I, Isovaleric adidaemia and Methylmalonic acidaemia. These also include information on necessary drugs, medical food and medical devices for carry-on luggage.
The E.S.PKU Metabolic Passport is not an official, legal document. It is intended as an auxiliary tool for border and security checks to quickly find the necessary information to assess the medication and food you are carrying. We recommend having supplementary manufacturer's declarations to hand.
How to fill the information on the E.S.PKU Metabolic Passport?
- Use a permanent fineliner or a biro. Do not use a washable pen or fountain pen.
- Fill in the front completely. If possible use the stamp of the hospital also in the section "name and address of the clinic"
- The name of "Allied Disorder" must be exactly the same as stated on this site
- Tick the appropriate boxes on the front and back
- When you have completed the front and before continuing with the back, wait a few seconds and allow the ink to dry
- Do not forget to indicate the dosages of your substitutes or medication on the back
- Do not forget to have the doctor's stamp and signature on the back, to verify the information provided
- Before touching the surface, wait a few seconds and allow the ink to dry.
- If you put the E.S.PKU Metabolic Passport in your official travel passport, you’ll have it at hand for passport and custom checks and security inspections
